Home Health Aide
As a Home Health Aide, you are responsible for performing personal care, respite, planning and preparing meals, laundry and light housekeeping.

To qualify as a Home Health Aide, you must meet the following criteria:

If you possess a GED or a high school diploma AND:
  • You are state tested; OR
  • You have completed a 75 hour training program; OR
  • You have a minimum of one year experience in the last three years, in a related field, such as a nurses aide or previously worked in home health industry.
If you do not have a GED or high school diploma, must have:
  • A minimum of two years experience in the last three years, in a related field, such as a nurses aide or previously worked in home health industry

Registered Nurse
As a Registered Nurse, you provide skilled nursing care to patients and are responsible for the total nursing care of any assigned patients in their homes.

To qualify as a Registered Nurse, you must possess a valid RN license in the State of Ohio and have one year related experience in the last five years.

Licensed Practical Nurse
As a Licensed Practical Nurse, you provide skilled nursing care to patients in their homes.

To qualify as a Licensed Practical Nurse, you must possess a valid LPN license in the State of Ohio and have one year related experience in the last five years.

Physical Therapist
As a Physical Therapist, you provide professional physical therapy services to patients as prescribed by physician in their homes.

To qualify as a Physical Therapist, you must possess a valid PT license in the State of Ohio, Baccalaureate degree in physical therapy and have one year related experience in the last five years.

Occupational Therapist
As an Occupational Therapist, you provide professional occupational therapy services to patients as prescribed by physician in their homes.

To qualify as a Occupational Therapist, you must possess a valid OT license in the State of Ohio, Baccalaureate degree in occupational therapy and have one year related experience in the last five years.

Medical Social Worker
As a Medical Social Worker, you provide professional social workers services to patients as prescribed by physician in their homes.

To qualify as a Medical Social Worker, you must be a Licensed Independent Social Worker, a Masters degree in social work and have two years related experience in the last five years.
